Patient Rights
You have the right to:
- Receive considerate, respectful, and dignified care that honors your personal values, beliefs, and preferences.
- Access treatment without discrimination based on race, color, religion, sex, gender identity, age, national origin, or disability.
- Expect medical decisions to be based solely on your health condition and available treatment options.
- Have your personal privacy respected.
- Receive care in a safe and secure environment.
- Exercise your rights without discrimination or retaliation.
- Know the identity and credentials of all individuals involved in your care.
- Be informed if your providers lack malpractice insurance coverage.
- Be notified of any physician ownership or financial interest in the facility.
- Request a change of physician, dentist, or anesthesia provider when possible.
- Receive understandable information about your diagnosis, health status, treatment options, and prognosis before care is provided.
- Be informed about any unexpected outcomes of care.
- Receive complete information from your physician about proposed treatments or procedures in order to give or withhold consent.
- Participate in decisions about your care and to refuse treatment, as permitted by law.
- Be informed of your rights before care is started or stopped, whenever possible.
- Receive information in a manner that fits your level of understanding, including interpretation or assistive services when needed.
- Involve your family or chosen representative in care decisions, according to your wishes and applicable laws.
- Receive appropriate pain assessment, management, and information about pain relief options.
- Give or withhold consent for any recording, filming, or photography for purposes other than your care, and request to stop at any time.
- Be informed of and consent to or refuse participation in research or educational projects.
- Expect confidentiality of all information related to your care and medical records.
- Access and request copies of your medical records within a reasonable time frame and request amendments if needed.
- Receive information about disclosures of your health information within a reasonable time frame.
- Have an advance directive (living will or power of attorney) and be informed of the facility’s policy regarding such directives.
- Receive information on fees, charges, and payment policies.
- Be free from restraints unless medically necessary, and never used for discipline, convenience, or retaliation.
- Be free from all forms of abuse, neglect, or harassment.
- Receive free language assistance from a qualified interpreter, if you have limited English proficiency or a disability.
- Expect the facility to have a process for prompt resolution of grievances, and know whom to contact to file a complaint.
If a patient is deemed legally incompetent, their appointed guardian or legal representative will exercise these rights on their behalf.
Patient Responsibilities
As a patient, you are responsible for:
- Treating staff and other patients with courtesy and respect.
- Following facility rules and instructions during your stay.
- Providing accurate and complete information about your health and medical history.
- Asking questions if you do not understand your treatment or instructions.
- Keeping appointments or notifying the facility if you must cancel.
- Arranging for a responsible adult to drive you home and stay with you for 24 hours if required.
- Fulfilling your financial obligations promptly.
- Informing your care team of any advance directive that may affect your treatment.
