Patient Rights

You have the right to:

  1. Receive considerate, respectful, and dignified care that honors your personal values, beliefs, and preferences.

  2. Access treatment without discrimination based on race, color, religion, sex, gender identity, age, national origin, or disability.

  3. Expect medical decisions to be based solely on your health condition and available treatment options.

  4. Have your personal privacy respected.

  5. Receive care in a safe and secure environment.

  6. Exercise your rights without discrimination or retaliation.

  7. Know the identity and credentials of all individuals involved in your care.

  8. Be informed if your providers lack malpractice insurance coverage.

  9. Be notified of any physician ownership or financial interest in the facility.

  10. Request a change of physician, dentist, or anesthesia provider when possible.

  11. Receive understandable information about your diagnosis, health status, treatment options, and prognosis before care is provided.

  12. Be informed about any unexpected outcomes of care.

  13. Receive complete information from your physician about proposed treatments or procedures in order to give or withhold consent.

  14. Participate in decisions about your care and to refuse treatment, as permitted by law.

  15. Be informed of your rights before care is started or stopped, whenever possible.

  16. Receive information in a manner that fits your level of understanding, including interpretation or assistive services when needed.

  17. Involve your family or chosen representative in care decisions, according to your wishes and applicable laws.

  18. Receive appropriate pain assessment, management, and information about pain relief options.

  19. Give or withhold consent for any recording, filming, or photography for purposes other than your care, and request to stop at any time.

  20. Be informed of and consent to or refuse participation in research or educational projects.

  21. Expect confidentiality of all information related to your care and medical records.

  22. Access and request copies of your medical records within a reasonable time frame and request amendments if needed.

  23. Receive information about disclosures of your health information within a reasonable time frame.

  24. Have an advance directive (living will or power of attorney) and be informed of the facility’s policy regarding such directives.

  25. Receive information on fees, charges, and payment policies.

  26. Be free from restraints unless medically necessary, and never used for discipline, convenience, or retaliation.

  27. Be free from all forms of abuse, neglect, or harassment.

  28. Receive free language assistance from a qualified interpreter, if you have limited English proficiency or a disability.

  29. Expect the facility to have a process for prompt resolution of grievances, and know whom to contact to file a complaint.

If a patient is deemed legally incompetent, their appointed guardian or legal representative will exercise these rights on their behalf.

Patient Responsibilities

As a patient, you are responsible for:

  1. Treating staff and other patients with courtesy and respect.

  2. Following facility rules and instructions during your stay.

  3. Providing accurate and complete information about your health and medical history.

  4. Asking questions if you do not understand your treatment or instructions.

  5. Keeping appointments or notifying the facility if you must cancel.

  6. Arranging for a responsible adult to drive you home and stay with you for 24 hours if required.

  7. Fulfilling your financial obligations promptly.

  8. Informing your care team of any advance directive that may affect your treatment.
